Fatality Accident

Location: Highway 275 and 541st Avenue

On April 29th, 2026, at approximately 4:18 a.m., a double fatality accident occurred on Highway 275 and 541st Avenue, in Madison County 2 miles East of Meadow Grove.

Through an investigation conducted on the scene, it was determined that a Blue 2018 Honda Odyssey traveling westbound on Highway 275 crossed over the center line. This caused a head on collision with a 2026 Peterbilt Tractor Truck hauling an empty tank trailer traveling Eastbound. The Honda spun around and came to rest facing Eastbound in the Westbound Lane. The Peterbilt came to rest facing Eastbound in a field on the Southside of Highway 275.

The driver of the Honda Odyssey, 51-year-old Lynn Phelps of Hastings, OK, and the passenger of the Honda Odyssey, 51-year-old Robert Phelps of Hastings, OK, were pronounced deceased on the scene. Next of kin has been notified.

The driver of the Peterbilt Tractor Truck , 71-year-old Michael Klasna of Emmet, NE was transported to Faith Health to be treated for injuries. 

Seatbelts were worn by all involved. 

Agencies involved were; Madison County Sheriff’s Office, Nebraska State Patrol, Meadow Grove Fire and Rescue, and Battle Creek Fire and Rescue. The accident is still under investigation by The Madison County Sheriff’s Office.

Intoxicated Driver on Highway 81 Leads to Arrest

Subject: Intoxicated Driver on Highway 81 Leads to Arrest

Location: Highway 81 and 835 Road

On 12/30/2025, at 10:00 a.m., Madison County Deputies responded to a traffic complaint on Highway 81, in Madison County. The original complaint started near the Platte Center area. The caller advised that the driver of a blue Toyota was all over the road and appeared to be falling asleep travelling Northbound.

Madison County Deputies located the vehicle on Highway 81 near 833 Road. The vehicle continued to weave into the adjacent lane nearly causing a collision with other vehicles. Deputies attempted to conduct a traffic stop, but the driver continued Northbound at speeds of 75 MPH. Deputies observed the male driver on his cell phone during this time. The male driver finally came to a stop on Highway 81 near 835 Road. It was also discovered that the male driver was not wearing a seat belt at this time.

The driver, identified as Hector J. Cash Hernandez (27,) of Columbus, was asked out of his vehicle by Deputies, and a DUI investigation was conducted. The investigation found Cash Hernandez to have a BAC of .185. The driver was placed under arrest and transported to the Madison County Jail.

This adds to the continued traffic hazards observed on Highway 81 over the recent months. Please continue to use safe driving habits and DO NOT DRINK AND DRIVE.

Stanton County Assist Leads to Arrest

Location: Intersection of N 1st and NE Industrial Highway

On the morning of 10/26/2025, Madison County Deputies were dispatched to the area of East Eisenhower. It was reported that a white male, wearing black pants, a red sweatshirt, and a black backpack was attempting to enter a residence in the area. 

As Madison County Deputies were enroute, Stanton County Deputies heard the radio traffic and advised they were 2 miles away. Given Stanton County was closer to the area, they were asked to assist and proceeded into Madison County.

As Stanton County attempted to have contact with the male suspect, later identified as 30-year-old, Chance D. Larsen of Pierce. Larsen ran northbound into a cornfield. Larsen ignored multiple verbal commands to stop during this time from Stanton County, and continued running through the cornfield. Due to this, a taser was deployed, making contact with Larsen’s lower back. Stanton County was able to detain Larsen at this time. 

Larsen was placed under arrest by Madison County Deputies. Larsen was booked into the Madison County Jail for the charges of First-Degree Trespassing, Obstructing a Peace Officer, and Resisting Arrest.

Hit and Run in Madison County Fair Parking Lot Leads to Arrest

Subject: Hit and Run in Madison County Fair Parking Lot Leads to Arrest

Location: Madison County Fair Grounds

On July 15th at 11:15 Pm, Madison County Deputies were dispatched to the Madison County Fairgrounds North Parking Lot to assist a female pedestrian who was struck by a vehicle.

The vehicle had left the scene of the accident, but witnesses were able to give a description of the vehicle, as well as a license plate number. Deputies intercepted the vehicle exiting the Fairgrounds on 6th street in Madison.

A traffic stop was conducted by Madison County Deputies that resulted in Brian L. Coffman, age 40, of Norfolk, NE being arrested for Driving under the Influence resulting in serious bodily injuries, Leaving the Scene of an Accident with Serious Bodily injuries, and Driving under Suspension.

The pedestrian was transported to Faith Regional to be seen for her injuries. Assisting Madison County was, Madison Police Department, BlueLine Security, and Madison Emergency Medical Services.
